MODEL OF CARE

The primary health team has a shared care practice, and as a team collaborates to provide high quality care to patients, and networks with external partners to advance clinical innovations. Patients are attached to a designated practitioner (either a family physician or nurse practitioner) most responsible for their care; care consistency is important if you have a complex or chronic medical condition. However, in receiving care from a team it also means you may see different providers at different times, depending upon the concern, and appointments may be longer or more in-depth.
