FPs and NPs have similar goals of providing excellent longitudinal care and promoting health and wellness. Both FPs and NPs diagnose and treat acute and chronic diseases and injuries. They examine patients, order and interpret tests and diagnostic procedures, prescribe medications, and perform minor surgical procedures. They identify health risks, address prevention issues, and manage end of life care. They work independently and collaboratively and refer to medical specialists.

Registered nurses assess, diagnose, triage, and carry out interventions within nursing scope, and have added responsibilities of patient intake, wound care, medication management, screening, immunizations, self-care education, and disease prevention.

LPNs provide direct patient care and can take vitals, collect specimens, administer medications, apply dressings & monitor patient progress. They offer health education in self-care to individuals or in group teaching.

Registered dietitians provide nutrition education and counselling to individual patients with chronic conditions, advise and consult with team members, and provide nutrition information and planning to the overall community.

Occupational therapists assess and determine therapy plans to help patients develop and regain physical, emotional, and mental functioning. They implement programs to foster activities of daily living and maximize mobility and independence according to patient’s physical capacity, intellectual status, and personal interests.

Medical office assistants assist patients to schedule appointments, triage urgency, and greet patients for care. They also facilitate patient flow, communicate team messages, manage clerical and administrative tasks, process patient documents, and maintain examination and supply areas.

The population health and wellness coordinator builds connections with community partners to address community gaps and bridge cultural and socioeconomic challenges, supports community participation and volunteerism, and develops health and wellness programs that foster community wellness.

As part of the Luther Court leadership, the director leads service growth and sustainability; manages team-practice including hiring, team learning and collaboration, and staff scheduling and supervision; oversees the day to day operations of care delivery, reporting and budgetary requirements, and service evaluation; and liaises with clinical and community partners.

Our mental health clinician supports case management, can provide emotional support and help patients with navigating social services and refer to community resources. The mental health clinician also supports community health with a focus on mental health and substance use, providing education and supporting our centre as a harm reduction site.
